GGleniUpdated October 2026

What is a chiropractic treatment plan?

A treatment plan is a written outline of the care you recommend: what you are aiming for, how you plan to get there and when you will check progress.

  • Goals: What the patient wants to achieve, in clear and measurable terms.
  • Proposed care: The types of care you recommend, such as adjustments, exercises and lifestyle advice.
  • Visit schedule: How often you suggest visits and for roughly how long.
  • Re-evaluation: When you will review progress and what might change the plan.

Sharing the plan helps patients understand what is involved and stay engaged with their care.

Who can use this treatment plan template?

Any practitioner who recommends a course of care, including:

  • Chiropractors starting a new course of care
  • Clinics standardising care plans
  • Associates and new graduates
  • Sports and rehabilitation practitioners
  • Family and paediatric practices
  • Practices that review plans regularly

Adapt the wording to your approach and to any rules your regulator sets about care plans and consent.

Why is a treatment plan useful for chiropractors?

A written plan helps in three ways:

  • Clear expectations: patients see the goals, schedule and review points up front.
  • Better engagement: a written plan helps patients commit to their care.
  • Measured progress: set goals and review dates make progress easy to check.

Benefits of using a chiropractic treatment plan template

Shared goals

Writing goals together keeps you and the patient aligned.

Fewer surprises

A visit schedule up front helps patients plan their time and costs.

Accountability

Review dates mean progress is checked, not assumed.

Smoother documentation

The plan links neatly to your exam and SOAP notes.

Patient confidence

A thoughtful plan shows a considered approach to care.

Commonly asked questions

Is a treatment plan a guarantee?

No. It is your recommendation based on current findings, and it can change as the patient responds.

How often should plans be reviewed?

At the interval you set on the plan, and any time the patient's condition changes significantly.

Should patients get a copy?

Many practitioners share a copy so patients can refer to goals and schedule. Follow your local rules about patient access to records.
