What is a chiropractic SOAP notes template?

A SOAP note records each visit in four parts: Subjective, Objective, Assessment and Plan. A template sets the sections out so every note follows the same format.

  • Subjective: what the patient reports, such as symptoms, pain levels and changes since the last visit.
  • Objective: what you find and do, such as movement, palpation findings, tests and the treatment given.
  • Assessment: your view of progress: how the patient is responding and any change in your impression.
  • Plan: what comes next, including visit frequency, home care advice, referrals and re-evaluation.

Repeating the same structure at each visit makes progress easy to follow and notes quick to write.

Who can use this chiropractic SOAP template?

Any chiropractic practice that documents visits, from solo practitioners to larger clinics. It works for:

  • Initial and follow-up visits
  • Adjustment-based care
  • Sports and rehabilitation care
  • Paediatric and family practices
  • Mobile chiropractors
  • Clinics with several practitioners

The structure is the same across techniques. The detail you record depends on your approach and your regulator’s guidance.

Why is a SOAP template useful for chiropractors?

A fixed structure helps in three ways:

  • Speed: filling in set sections is faster than writing free text.
  • Consistency: every visit is recorded the same way, so notes are easy to compare.
  • Clarity: separating what the patient says from what you find keeps records clear.

Commonly asked questions

How detailed should SOAP notes be?

Detailed enough that another clinician could understand the visit, but brief enough to complete in a few minutes. Follow your regulator's guidance.

Can I use SOAP notes for every visit?

Yes. Many practices use a full initial exam at the first visit, then SOAP notes for follow-ups.

Do privacy laws apply?

In many regions they do, because notes contain health information. Check the rules where you practise. Ruana is designed with HIPAA and GDPR requirements in mind.
